Lip filler migration is one of the biggest concerns clients raise during consultation. While no treatment is completely risk-free, there are several ways we can minimise the risk and prioritise long-term lip health.
What Is Lip Filler Migration?
Lip filler migration occurs when dermal filler moves beyond the natural borders of the lips into the surrounding tissue. This can create a less defined appearance and may cause the lips to look puffy, heavy or "shelf-like", particularly above the upper lip.
It's important to understand that lip filler migration doesn't happen to everyone, and having lip filler does not automatically mean your filler will migrate. In fact, many clients enjoy beautiful, natural-looking results for years without experiencing any issues.
Migration is thought to be influenced by a number of factors, including the amount of filler used, how frequently treatments are performed, individual anatomy, previous filler history and the techniques used during treatment. In some cases, migration can be very subtle and only noticeable during a detailed assessment, while in others it may be more obvious.

What Causes Lip Filler Migration?
Overfilling - The lips can only hold so much filler until it has to travel past the lip border into the surrounding tissue
Frequent Top-Ups - Constant top ups don't allow the lips time to adjust to the new volume, resulting in the filler migrating
Individual Anatomy - Fine lips must be treated differently to naturally fuller lips, for example, not placing filler in the extremely fine areas like the outer corners
Poor Technique - If filler is placed in the wrong plane of tissue or too much filler is deposited in one area, its far more likely to move
Ignoring Existing Migration - Once those migrated channels have been created, adding more filler without dissolving will worsen the problem
How We Minimise The Risk Of Lip Filler Migration
1. We Don't Overfill
At NTK Aesthetics in Manchester, we believe that less is often more. While it can be tempting to continually add volume, overfilling places unnecessary pressure on the tissues and may increase the risk of migration over time. Our aim is always to create lips that are balanced, proportionate and in harmony with the rest of your features, rather than chasing trends or excessive volume.
2. We Often Recommend 0.5ml
Bigger isn't always better. For many clients, particularly those new to lip filler, 0.5ml or even less is enough to subtly enhance the lips while maintaining a natural appearance. Building volume gradually allows us to assess how your lips respond to treatment and helps reduce the risk of overstretching the tissues. Beautiful lips are built over time, not in one appointment. Often for finer lips we start with 0.3 and build up to avoid migration as much as possible.
3. We Carefully Assess Previous Filler
Migration doesn't always happen overnight. In some cases, filler may have been slowly accumulating for years, especially if treatments have been performed frequently or by multiple practitioners. This is why every appointment starts with a thorough assessment of your lips and previous filler history. Sometimes the safest option is to pause treatment rather than continue adding more filler.
4. We Recommend Dissolving Where Appropriate
More filler isn't always the answer. If we feel existing filler has migrated or is compromising the final result, we may recommend dissolving before starting again. Although this can feel disappointing, taking a step back and starting with a clean canvas often leads to healthier tissues and more natural, long-lasting results in the future. 5. We Allow Enough Time Between Appointments
Lip filler needs time to settle and integrate into the tissues. Frequent top-ups can lead to a build-up of product over time and increase the likelihood of an overfilled appearance. We encourage our clients to wait until their filler has naturally reduced before considering further treatment, helping to maintain long-term lip health.
